What Is Family Supports Waiver (FSW)?

The Family Supports Waiver (FSW) is an Indiana Medicaid program that provides essential services to individuals with developmental disabilities who live with their families or independently. It offers respite care, personal assistance, therapy, and behavioral services to help individuals maintain independence and community involvement. The FSW is designed to enhance quality of life while reducing the need for institutional care.

Eligibility Criteria for the Family Supports Waiver (FSW)

  1. Diagnosis Requirements:

    • Must have a documented intellectual or developmental disability (e.g., autism, cerebral palsy) or a related condition with an onset before age 22.
    • The condition must be expected to continue indefinitely.
  2. Level of Care:

    • Must meet the Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID) level of care.
    • This includes substantial functional limitations in at least three of the following six major life areas:

      • Self-care
      • Learning
      • Self-direction
      • Capacity for independent living
      • Understanding and use of language
      • Mobility
  3. Medicaid Eligibility:

    • Must be eligible for Medicaid.
    • For individuals under 18, parental income is disregarded; only the child’s income and assets are considered.
    • For individuals 18 and older, income must not exceed 300% of the maximum Supplemental Security Income (SSI) amount.
  4. Residency:

    • Must reside in, or be transitioning into, a home and community-based services (HCBS) compliant setting (non-institutionalized).
